Abstract	Section 208 of the Energy Reorganization Act of 1974 identifies an abnormal occurrence as an unscheduled incident or event which the Nuclear Regulatory Commission determines to be significant from the standpoint of public health or safety and requires a quarterly report of such events to be made to Congress. This report, the twelfth in the series, covers the period January 1 to March 31, 1978. The NRC has determined that during this period there were two abnormal occurrences. One occurred at the 68 nuclear power plants licensed to operate and involved insulation failures in containment electrical penetrations. The other occurred at an Agreement State licensee and involved an overexposure of a radiographer. This report also contains information updating previously reported abnormal occurrences.
