Abstract	This report documents the work conducted in the development of an attitude survey for Contract 211-93-0006, Produce a survey to assess pre- and post-intervention the attitude of workers toward the use of occupational safety equipment and toward protecting themselves from occupational noise-induced hearing loss. The contract was conducted in four phases. During Phase I, a detailed work plan for the project was prepared and approved. This work plan included an item specification plan which detailed the content areas that the survey would address. In Phase II, the item pool was developed and finalized. In addition, an item tryout was conducted by administering the item pool to a sample of construction workers. Analyses of the response data permitted the development of two forms of the final survey. Phase II ended with the collection of test-retest reliability data for the survey. In Phase III, additional test-retest data were collected. The final report was prepared in Phase IV.
