Abstract On March 20, 1990, the Vogtle Electric Generating Plant Unit 1, located in Burke County, Georgia, about 25 miles southeast of Augusta, experienced a loss of all safety (vital) ac power. The plant was in cold shutdown with reactor coolant level lowered to 'mid-loop' for various maintenance tasks. Both the containment building personnel hatch and equipment hatch were open. One emergency diesel generator and one reserve auxiliary transformer were out of service for maintenance, with the remaining reserve auxiliary transformer supplying both Unit 1 safety buses. The report documents the results of an Incident Investigation Team sent to Vogtle by the Executive Director for Operations of the U.S. Nuclear Regulatory Commission to determine what happened, identify the probable causes, and make appropriate findings and conclusions.
