Abstract An analytical planning methodology is developed and applied to an urban emergency medical system to evaluate the gap between clinical advances in emergency medical care technology and the level of care typically rendered in life-threatening emergencies. The objective of the methodology is to reduce mortality and disability that result from emergencies. It is intended to provide a systematic technique for the identification of appropriate allocation schemes for emergency medical care resources. Two mathematical models constitute the methodology: a semi-Markovian representation of the acute disease experience of a target population, and a representation of relationships between the services provided by components of an emergency medical system and the costs of operating the system. The results of the methodology's application to cardiac care emergencies in Allegheny County, Pa., are presented. It is suggested that the allocation of a high level of emergency cardiac care capability may not be cost-effective. Consideration is given to the extent to which operations research practitioners are limited by the availability of actual data in the investigation of complex social problems and whether knowledgeable experts can construct management solutions to such problems. Potential areas for further research are identified. Additional information on the application of the methodology in Allegheny County including tabular data is appended. A bibliography is provided.
